Subpoenas must be mailed or delivered in person to Corporation Service Company (CSC), ChristianaCare’s registered agent, at 251 Little Falls Drive, Wilmington, DE 19808. Outside of Delaware, subpoenas should be sent to CSC in the state of the facility they are seeking to subpoena.
For patients or their legal representatives to request a copy of the patient’s own medical record, the Request for Access form should be printed, completed and sent to the Health Information Management Release of Information Department at the address below. The charges for hospital services include general nursing care, visits by hospital staff physicians, housekeeping, maintenance, medical supplies, laundry and linens, meals, administrative services and room charges.
