myChpCare is a free service that provides secure, online access to your health information and medical health records. To access myChpCare, you need access to the Internet, an email address and personal information to help identify yourself.
Capital Health Medical Group offers two free online patient portals to give you access to your health information and information about any of your visits to our offices. One portal provides information for current visits. One portal provides information for visits prior to September 13, 2016. If you have any issues logging into either portal, please contact your doctor's office during regular business hours.
