We have 6 locations on the MidShore (Denton, Goldsboro, Cambridge, Easton, and St Michaels) and will soon be adding a new site in Chestertown. The Behavioral Health Clinician will work collaboratively with a diverse team of providers, to help shape the new integrated Behavioral Health Program at Choptank Community Health. Under the direction of the Director of Behavioral Health, and as a part of a multidisciplinary team the Behavioral Health Clinician will be responsible for providing individual (children and adults), couple, family and group therapeutic services, Behavioral Health Consultations and Brief Interventions for patients referred by Choptank’s PCP’S and Dental Providers.
The Community Health Worker (CHW) is responsible for acting as a bridge between health care providers, community services providers, and individuals in the community to promote health, reduce disparities, and improve service delivery. The Community Health Worker is a trained and trusted public health worker who is respected by the people they serve and applies his/her unique understanding of the experience, socio-economic needs, language and/or culture of the communities served. CHWs identify barriers to health and advocate for changing service delivery and other strategies to improve community health.
The School Based Dental Program (SBDP) Dental Hygienist is a critical part of the CCHS oral health team and plays an essential role in the prevention of dental decay in children. Provides dental prophylaxis, fluoride varnish treatment, dental sealants and oral hygiene instruction. Plans and conducts oral health educational programs within the school setting with a focus on the prevention and control of dental disease. Initiates patient referrals for follow up care and serves as a key link between dental providers and patients/families. Assists in the coordination of the day-to-day SBDP activities including record maintenance and other activities as needed.
The Patient Services Specialist’s is often the first point of contact a patient has with CCHS and represents all the best qualities of CCHS by providing every patient with truly exceptional services. Only a special few truly have the internal drive to perform their job with the utmost care and compassion our patients deserve. While prior medical office experience is beneficial and preferred, we consider genuine customer service delivery and a work history which demonstrates reliability and tenure among our highest priorities.
Prepares, sterilizes organizes and sets up equipment. Ensures all dental instruments are clean, dated and processed according to established clinic guidelines. Works with community groups and organizations to offer education programs that are age/audience appropriate and contain current oral health research/topics.
The Medical Director of Behavioral Health contributes to the successful achievement of the Choptank Community Health System (CCHS) mission and strategic vision by providing clinical leadership and oversight of the Behavioral Health service line, including providing clinical supervision to all psychiatric medical providers, coordinating with CCHS’s other health service lines to ensure holistic patient-centered care, and helping to build a culture of clinical excellence and continuous performance improvement . The Medical Director of Behavioral Health will also participate in the assessment, screening, diagnosis, and treatment of patients, and collaborate with other CCHS clinicians to discuss treatment plans and progress. This position requires clinical expertise in providing behavioral health services to indigent populations and working in community settings. The selected candidate she should also have strong management, problem-solving, and communication skills.

Maintains knowledge of trends, best practices, regulatory changes, and new technologies in human resources, talent management, and employment law.
