Crawford Memorial Hospital and the CMH Clinics are pleased to announce they have a PATIENT PORTAL to allow you to communicate with us easily and safely - according to your schedule. Using your secure password, you can log into the online patient portals 24 hours a day, 7 days a week from the comfort and privacy of your home or office.

MyChart provides an intuitive, secure, web-based method for patients to access portions of their medical records through our secure web site.

Where are the terms and conditions on MyChart?

Terms & Conditions can be accessed while logged in to the MyChart Patient Portal. From the Home page, use the Terms & Conditions link at the bottom of the page.

What to do if you find errors in MyChart?

If you find errors in your personal health information on MyChart patient portal, please contact your physician's office, and they will consult with you to resolve the error. It is a priority of ours to maintain an accurate record of your health care.

The ministry isn’t a profit-seeking enterprise. Instead, our purpose is to glorify God and serve His people; our members are our sole concern. 2 CHM members are in a covenant—not contract—with each other. Members are Christians living by biblical principles and voluntarily join together to share each other’s medical costs. CHM shares 100 percent of all bills that are eligible according to our Guidelines. 3 Program costs remain the same regardless of your health history; no physical evaluation or medical tests are required to join.
