How do I login to my child's patient portal?

Once your child is registered for care at Shriners Children's, you will receive an email with login instructions to access your child's patient portal. The portal can be accessed online through your browser via the link below or through the complimentary HealtheLife app which can be downloaded from the Apple App Store or Google Play Store.

What information can I find in the patient portal?

When you log in to the portal, you will have access to: Appointments, lab results, allergies, vital signs, provider notes and your child's summary of care. You can request a copy of your child's medical record from the Patient Portal.

Who can see my mychkd account?

MyCHKD account holders should be aware that they will be notified by e-mail for certain features, including activation. This means that any person with access to their e-mail inbox will be able to see this notification. Can anyone have a MyCHKD account? MyCHKD is offered to parents/legal guardians of children ages birth to 14.

What are the aspects of patient care?

Comprehensive aspects of patient care including referrals, medication management, diagnostic tests, immunization administrations and other services are provided by the practice.

How to access pediatrics east?

To access your child's patient portal, you must be registered by Pediatrics East. To register, you must either request a registration in the office or call our office. Each child in the family must have a separate account. Once you have been registered by Pediatrics East, you will receive an email confirmation with a username and temporary password. Please allow 24 hours for your account to be activated once your child has been registered.

How to return a medical form to a physician?

Return the forms to your physician by dropping them off at the office, by mail, or by fax (addresses and phone numbers are at the top of the page). Note, it may be easier for the parent to collect the forms from the teachers and return all of the forms together at once.

The first step in getting your child evaluated for Attention Deficit and Hyperactivity Disorder (ADHD) or Attention Deficit Disorder (ADD) is to talk with your child’s teachers at school and see if their concerns match with your concerns on his/her behavior at home.

Pediatrics East was proud to receive Patient Centered Medical Home certification from the National Committee for Quality Assurance (NCQA) in 2013. The NCQA Patient Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. We were recertified in 2021.

Do expecting mothers get free site visits?

We offer free expecting mother/parent site visits. This will give you the opportunity to visit our clinic, meet with one of our physicians and ask questions regarding our practice.

Can you fax medical records to Pediatrics East?

This form authorizes Pediatrics East to fax medical information such as excuses, medication lists, and sports physical forms to any person or party (such as your child's school or your job) that you want information sent. No medical information will be directly sent to anyone other than a parent or guardian without this form completed. Please note that this is NOT a transfer of records form. If you are wanting to transfer your child's medical records from another doctor's office to our practice, please use the forms below.

What to do if you have not received your Shriners patient portal password?

If you have not received your username and password for the Shriners Patient Portal, please contact your child's care manager at the location responsible for your child's care. They will be able to provide you with the login information.

How to get a copy of my child's medical record?

To do this, just log in to their portal and review the Request for Medical Record Release form for instructions on how to submit your request.

What do you communicate with the care management team regarding?

You may communicate with the care management team regarding routine issues or questions related to your child.

Where to register for MyCHKD?

Parents/legal guardians must register for a MyCHKD account in person at the pediatrician’s office. This only takes a few minutes. After that, you will receive an e-mail at the address you provide with instructions on how to set up an account. Please note: if you provide a shared e-mail address, anyone you share that address with will also be able to see the e-mail.

MyChildren’s is a convenient, easy-to-use, and secure way to access your child’s health information. Using MyChildren’s, you can: view portions of your child’s health record. send secure messages to your care team. view and update your contact information. view and pay your bills online.

How to update insurance information?

To update your insurance information, go to Billing and select Insurance Summary. Please note that we will review all insurance changes.

How long before a doctor visits you can confirm?

Yes, you can confirm visits up to 30 days prior to your visit.

How to contact MyChildrens?

For help with the portal, you can fill out our support form at mychildrens.org/help or call the MyChildren’s Support Team at 617-919-4396 Monday to Friday from 7 a.m. to 7 p.m. and Saturday from 8 a.m. to 4 p.m.

When can parents access their child's information?

Parents will no longer have access to their child’s information once the patient turns 18. The patient cannot grant access to a parent to restore access. My child with special needs is over the age of 18.

When are diagnostic tests available?

Diagnostic test reports are available as soon as they are resulted.

I am enrolling for the first time

You can enroll in the patient portal online or through printed forms. We'll provide directions for both.

I am enrolling another child

If you have a patient portal account for one child, you can add additional children to your account. We'll help you get started.

My child is over 18, I need enrollment or access information

We can help you get access to patient portal accounts for patients over the age of 18. There are typically a few situations where this is needed:

What can I do with the Patient Portal?

Users can connect to the child’s information from Phoenix Children's, and also access the child’s health information from other health institutions that have partnered with FollowMyHealth.

Looking for Immunizations?

Immunizations are no longer available in Phoenix Children’s patient portal at this time. To access immunization information visit the Arizona Department of Health Services (ADHS) webpage. Please note this may not be a complete immunization record due to limitations with reporting to ADHS.
