Your patient portal will include components of your medical record from St. Vincent’s hospital stays after December 2, 2017. It only takes a few minutes and four simple steps to set up your personal patient portal, available anytime from your computer, tablet or mobile device!

Is there a paper document required to access the patient portal?

This system is paperless and patient information will be accessible via the portal. If a paper document is required or you wish to delete your patient portal account, please contact 619-662-4100.

How do I sign up for the Ascension patient portal?

You will immediately receive an email from Ascension Patient Portal <noreply@iqhealth.com>; click the secure link in the email. Follow the online instructions to set up your username and password.

What to do when you are discharged from the hospital?

When you are discharged from the hospital, someone will talk with you about which medicines to take and which medicine to stop taking. Since changes are often made after a hospital stay, a new form should be filled out. When you return to your doctor, take your new form with you.

Why do we keep a list of all the medications we take?

To reduce medicine errors and keep patients safe, you are encouraged to keep a list of all the medications you take (including prescription, herbals, over-the-counter and vitamins) and bring that list to all doctor visits, hospital visits, and also when you go in for tests or visit other healthcare providers.
