Health Care Online. MyChart is a secure, online tool connecting you to parts of your medical record and allows you to manage your health care when it is convenient for you – 24 hours a day. It is a free service offered to our patients. To access the patient portal, you will need computer access with an internet connection or a smartphone.

The patient portal invitation you receive after visiting one of our facilities will never expire. Once you sign up, the portal will be available to you 24/7.
Patients who wish to participate will be issued a MyChart activation code during their clinic visit. This code will enable you to log in and create your own username and password. If you were not issued an activation code, you may call your primary care clinic to get one, ask to sign up during your next office visit or sign up online.
MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to:
You will generally receive an answer within 1-3 business days. Please note that MyChart should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.
Your test results are released to your MyChart account after your physician has reviewed them. This is generally within 5 business days.
While logged into MyChart, if your keyboard remains idle for 15 minutes or more , you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time. Return to Top.
TTo request a new activation code on MyChart, you may call your primary care clinic to get one, ask to sign up during your next office visit or sign up online.
Simply go to the App Store or Google Play and search "MyChart Powered by Epic" and install the app on your tablet or mobile device.
Currently available is the Health app, which is offered by Apple Inc. for iPhones with iOS version software 11.3 or greater. You will recognize it easily by the heart image. The Health app consolidates all of your health data from iPhone, Apple Watch, and third-party apps you already use, so you can view all your progress in one convenient place. This includes your health records from CHI St. Alexius Health and other health systems if you choose to add to add them. To get started:

Mid Dakota Clinic, a member of PrimeCare, but not a CHI St. Alexius Health owned clinic, maintains their clinic records at the Mid Dakota Clinic. If you need records for clinic care provided at Mid Dakota, please contact the Mid Dakota Clinic Release of Information Nurse.
