When you arrive at Cheshire, go directly to Family Medicine on Level 1A North via the central elevator or staircase. Bring the following:
Your primary care provider will refer you to a specialist. The office of that specialist will contact you to set up your appointment. If you have a myD-H Patient Portal account, you can log into your virtual visit up to half an hour before it begins right from your appointments or visits page in myD-H or the MyChart app. Sign up for an account at myD-H.org before your visit.
