How to Enroll in the Patient Portal: 1 You will need to get an enrollment token from your Health Center. You can either call or stop into your Health Center and ask for enrollment token. Your enrollment token will be a series of numbers. 2 Click on the Patient Portal Login box above. 3 Enter your enrollment token that you were given from your Health Center, and enter your email address. You will also need to enter your birth date.#N#*The email address you enter must be the one you provided to the Health Center when you received your token.#N#*If you do not have an email address, you will need to enter your last name. 4 Choose a username and password.#N#*Your username and password are case sensitive.#N#*Usernames must be 6-20 characters.#N#*Passwords must be 6-20 characters and contain at least one (1) number. 5 Choose a login security question from the list provided, and provide your answer for the question.#N#* The question you select will need to be answered every time you log into your Portal account, due to your account containing confidential medical informationthat must be protected. 6 Set up your accounts secure password reset feature.#N#* The Patient Portal will ask you to select a question and provide an answer.#N#* If you forget your password, the Portal will be able to reset your password by answering the selected question.

Reach Out and Read

Eight of our locations participate in Reach Out and Read, a national evidence-based program that promotes literacy through pediatric care. At every appointment, from newborn through age five, children receive a brand-new, developmentally appropriate book to take home.

Women Infants and Children Program (WIC)

Cherry Health collaborates with the Kent County Health Department to offer the Women, Infants and Children Program (WIC) at Cherry Street Health Center and Westside Health Center. This is a state-funded nutrition education and food supplement program for pregnant, breastfeeding, and postpartum women and children up to age five.

Maternal Infant Health Program

The Maternal Infant Health Program (MIHP) is a parent support and coaching program for pregnant mothers and infants. If you are pregnant or have a baby under a year old, and are Medicaid eligible, you can receive MIHP as part of your insurance coverage.
