What is the CHC patient portal?

CHC/SEK’s Patient Portal is a secure website that allows patients to view their personal health records, including labs, diagnostic imaging, immunization records, upcoming and historical appointments, visit summaries, and education. You can also view questionnaires, messages from staff, billing statements, and demographic information.

What services does the CHC/SEK clinic provide?

You can call us 7 days a week and speak to one of our experienced, friendly staff who will be able to help you. Our hours are: Monday-Friday: 7:30 am-8:00 pm. Saturday: 8:00 am-5:00 pm. Sunday: 12:00 pm-5:00 pm. FIND A LOCATION.

Can I use the patient portal for emergency-related health care?

Information for new and existing patients

If you need to speak to a medical provider outside of our normal operating hours, we have after hours call numbers for every location. These numbers are not to be used for emergencies – if you are experiencing a medical emergency, please call 911.

How to apply for a new patient?

There are a few things you’ll need to bring to your first appointment: 1 Your insurance card. 2 Proof of income (in the form of pay stubs or tax return) if you are applying for discounted services. 3 A list of all medications you are taking (or bring the medications with you), and your vaccination history. 4 Your New Patient Information Form.

How old is a minor in CHC?

Patient Portal: CHC will automatically deactivate portal access for parents and guardians of minors between the ages of 10 through 17 years old. Parents may contact CHC for billing information or medical/dental records for their minor child and continue to have information about their health care

Athena is our patient portal for Medical services and NextMD is for our Dental services.

Can you use the Patient Portal for emergency care?

No, the Patient Portal is not to be used for emergency-related health care issues. Please call your Provider’s office directly for any questions you would need answered within 3 business days or sooner. If you are experiencing a health care emergency, call 911, or visit the nearest emergency room.

Where does health information come from?

This information comes directly from your medical record at your provider’s office. Discuss any inaccurate information with your provider at your next clinic visit. Your health information is reviewed and updated in your medical record during each visit.

Can a CHC give you access to your child's health care without written authorization?

These laws also require health care providers to treat visits confidentially. Therefore, if your child consents to one of the above services on their own, CHC will not be able to share information with you regarding those visits without written authorization from the child. These are the steps

Who handles a provider's request?

Designated staff members who provide support to your provider will be involved in receiving your messages, and routing them to your provider, nurse, or front desk as necessary. If your provider is out of the office, your request may be handled by the provider on-call or held until he/she returns to the office.
