If you currently have Proxy Access to your child's Catholic Health MyChart account, as of their 12th birthday the access will change to "limited" access. Access to view Allergies, Immunizations, Preventive Care, Health Summary, Medical History and Hospital Admissions will continue. Your child (over age 12) may request their own MyChart account.
