Chapa-De offers a sliding fee scale for uninsured and low income patients depending annual income and family size. This discount fee scale is for Non-Indian patients, and is based on Federal Poverty Guidelines. We have a 50% or 75% sliding scale, which is the portion of the charges that the patient must pay.

As a part of these standards, Chapa-De provides you the following “Notice of Privacy Practices” about how your Protected Health Information (PHI) may be used and disclosed in our healthcare practices as well as how you may access this information.
All new patients must register before their first appointment, please see How to Register. New patients should arrive at the reception desk in the department you are visiting 30 minutes before your first appointment to complete the new patient registration process. Established patients should arrive 15 minutes before your appointment time.
You will need to bring the following items to all of your appointments: 1 Photo ID 2 All insurance cards (Medi-Cal, Medicare or other medical, prescription or dental insurance). 3 A list of medication you are taking or bring the medications with you. 4 The patient services representative making your appointment will also inform you of any records or information you need to bring with you. 5 When bringing children in for immunization, always bring their immunization record with you.

Family PACT provides family planning services, such as birth control pills, IUDs, vasectomies, and education to low income men and women of child bearing age. Chapa-De accepts Medi-Cal and Medicare insurance plans. We accept both Medi-Cal Managed Care plans that are available to patients in our area, Anthem Blue Cross and California Health and Wellness, as well as straight Medi-Cal.
One of the newest members of the Chapa-De team is Manena Ng’ambi, the Diabetes Program Manager. Manena joined Chapa-De earlier this year. As Program Manager, Manena oversees the Diabetes Program and works with team members to provide excellent care to patients.
