What is a CGFM?

Patient Portal – We recently switched records systems to Providence’s Epic (CGFM is still an independent organization) and our old patient portal is no longer available. Please ask us to sign you up for MyChart to access your test results, after visit summaries, and billing information. Mental Health Support – Times have been stressful.

How do I Become A CGFM?

Patient Forms: Patients are asked to provide (and periodically update) a detailed medical history, health screen, and consent forms when under the care of Columbia Gorge Family Medicine. Appointments. At Columbia Gorge Family Medicine, we have experienced Doctors, Nurse Practitioners, and Physician Assistants who are dedicated to caring for you and your family. Make an appointment with your provider during our regular appointment hours, or stop in for an after-hours visit for any acute care needs.

How do you become a CGFM?

To earn the CGFM, you must fill out an online application and meet the following requirements:

TOL Secure Patient Portal

The TOL Patient Portal (also referred to as "TRICARE Online" or "TOL") is the current secure patient portal that gives registered users access to online health care information and services at military hospitals and clinics.

MHS GENESIS Patient Portal

MHS GENESIS is the new secure patient portal for TRICARE. It will eventually deploy to all military medical and dental facilities worldwide and replace the TOL Patient Portal.

Using MHS GENESIS and TOL Together

If you’re already a registered user on the TOL Secure Patient Portal, MHS GENESIS works much the same way.
