You can notify CareCentrix of new information by: clicking here or visiting www.carecentrix.com/patients/change-of-address. calling us at 1-800-808-1902 and follow the prompts for billing. Please allow 24-48 hours to have your account information updated within our systems.

Does CareCentrix provide diabetic supplies?

Yes. If CareCentrix is contracted with your health plan to provide diabetic supplies, CareCentrix will need to know the frequency of your testing and the specific brand of supplies requested. Please have your doctor fax this information to: 1-800-700-2085.

FollowMyHealth encrypts your data while it is in transit and encrypts it again when it is stored in the database for your use. Maintaining your privacy is our top priority and your information cannot be accessed without your user ID and password.
