What is a patient care portal?

Before you can take advantage of everything the Patient Portal has to offer, you must have an active account. To get access, you will need to contact your primary care provider. Prescription Policies and Information

Please note: In an effort to provide better care and more efficient service, we are changing our prescription refill request policy. Effective Tuesday, June 1, 2021 we will no longer accept faxed prescription refill requests from pharmacies.

Missed Appointment Policy

We require appointment cancellations within 24 hours so that we can schedule other patients that require medical attention. Appointments that are not cancelled within 24 hours may be assessed a charge of $50.00. If three (3) appointments are cancelled without a 24-hour notice, this may result in discharge from our practice.
