How do I get my medical records from CentraCare?

Individuals may submit completed and signed authorization forms to us through mail, fax or by email at CentraCareRecordRelease@centracare.com. Records will be returned through MyChart when possible or through alternative means as indicated. Patient pickup will not be available at this time.

How do I update my personal information with centra Medical Group?

Please update your personal information at your next visit to the hospital or any Centra Medical Group practice after September 1, 2018. You can continue to access your old records by clicking on the “Click to access previous data” link below the HealthyU logo on the centrahealth.com website.

How do I verify my identity as a Centra patient?

We appreciate everyone's patience during this transition and thank you for choosing Centra as your healthcare provider. Enter the information requested including your first name, last name, and date of birth. Click on the “Email address” tab and enter your email address. Check the “I’m not a robot” box in Identity verification and follow prompts.
