What is the Center for health care services?

The Center for Health Care Services (CHCS) provides release of information services for patients (or their parents/legally authorized representative) who request copies of their medical records from CHCS. Requests for medical records are not handled by staff at CHCS clinics.

How to receive services from the Center for Health Care Services?

To begin receiving services with The Center for Health Care Services, there is a series of screenings to determine eligibility. Whether it is on the phone or in-person, the first step is to complete a short screening to check how you are doing in the moment – are there more intense needs that we can take of right now?

What is CHCS treatment?

CHCS embraces a comprehensive approach to treatment and recovery. Our outpatient treatment model brings other specialties and disciplines into the treatment team to help you achieve your recovery goals. Medication may help manage symptoms while you work with your treatment team to build and expand coping skills and self-care strategies for recovery.

How long does it take to get a CHCS?

This process may take up to 3 hours. If he/she qualifies for services, the clinician will work with the individual to establish outpatient services at a CHCS clinic. If they do not qualify for services, the clinician will provide an appropriate referral in the community.

Does CHCS accept Medicaid?

The Center for Health Care Services accepts many insurance plans including Medicaid and Medicare. Payment of your estimated consumer portion is expected at the time services are provided. This includes deductibles, copays, and any other predetermined fees due for this visit. CHCS will not refuse services to a person based on inability to pay. You may qualify for financial assistance towards your payments. This qualification will be determined based on your income and family size. In order to determine the appropriate monthly fee, you must provide required documents and complete a Financial Assessment. Please be prepared to show proof of insurance, proof of residency and family income upon check-in.
