What is the new patient portal for CentraState?

Starting June 22, 2021, CentraState’s physician practices will be moving to a new electronic health record system and new patient portal, MyChart. At that time, access to your practice’s portal account will be limited to medical records for visits that occurred before June 22.

How do I get access to CentraState Medical Center medical records?

CentraState Medical Center patients have access to our free and secure online patient tools allow you to easily track your medical history, handle routine health management, and communicate with your physicians. Get access to medical records by filling out the request form online. You can request medical records for yourself or someone else.

732-294-2707 from outside the hospital OR Ext. 2707 from an in-house phone. CentraState’s care coordinators are certified nurses who support patients, their families, and other caregivers by coordinating and monitoring care from pre-admission to post-discharge from the hospital.
