If you are seen at a participating hospital and provide your email address upon registration, you will automatically receive an email invitation following your visit. Once you receive the email, follow the prompts to create your account and establish the connection with the appropriate physician, practice or hospital.

How old do you have to be to access CCHC?

Parents have the ability to view their child’s health information, within the CCHC patient portal, for children up to age 10. Due to various state and federal laws regarding sensitive and private health data for minors, CCHC is configured to allow parents access to his/her child up to, but not including, age 10. Once the child turns 10, the parent will no longer be able to access the child’s record in CCHC – and will need to contact the healthcare provider directly to request copies of records.
