How to get CCHC invitation?

Contact your participating physician’s office within Monterey County and request a CCHC invitation through your email address. If you are seen at a participating hospital and provide your email address upon registration, you will automatically receive an email invitation following your visit. Once you receive the email, follow the prompts to create your account and establish the connection with the appropriate physician, practice or hospital.

How old do you have to be to access CCHC?

Parents have the ability to view their child’s health information, within the CCHC patient portal, for children up to age 10. Due to various state and federal laws regarding sensitive and private health data for minors, CCHC is configured to allow parents access to his/her child up to, but not including, age 10. Once the child turns 10, the parent will no longer be able to access the child’s record in CCHC – and will need to contact the healthcare provider directly to request copies of records. Please call or email the CCHC Help Desk and ask for assistance with adding a “dependent account” to your current primary CCHC patient portal account if you have a child under 10.

How to opt out of CCHC?

If they choose, your doctor and other care providers will still be able to use the Health Information Exchange to have your lab results, radiology reports and other data sent directly to them; previously, they may have received this information by fax, mail, or other electronic communication. What is HIE in healthcare?

The HIE stores and transmits various health data that is critical to the care and treatment of patients. Depending on the participating organization from which you receive care, this can result in lab results, transcribed/dictated reports, radiology reports, emergency department discharge notifications and health summary documents. How to join CCHC?

Call or visit your physician’s office and request an invitation to join CCHC. If you have a visit at a participating hospital organization and provide your email address at registration, you will receive an invitation following your stay.

Can you view CCHC results electronically?

You will only be able to view results electronically in CCHC that was completed since the implementation of CCHC (i.e. auto-release of results were enacted) and to contact the Montage, SVMH, or NMC HIM departments to request to view any prior information.

Step 1: Confirmation Email

Check your email inbox for the confirmation message to begin the registration process. The message will be sent from 'donotreply@cchc.com'. Click the link provided to create a new user.

Step 2: Registration

You will be asked to enter your name, date of birth, unique user name and password. Important: Make sure you check the box to the right to verify your information is correct. Click the "register" button at the bottom of the screen.

Step 3: Security Questions

Choose three security questions with the answers, then click Submit. On the next page, click "Main Menu" to view your clinical records.

Step 4: Patient Records

From the patient list, click on the name of the patient you are authorized to view. The patient's clinical visits will be listed chronologically. Next to each visit is a link containing the account number and the title "Summary of Care". Click on the link in the Summary of Care column to view the document.

How to log into PCHC portal?

When you want to log on to the eHealth Patient Portal, go to www.PCHC.com and click on the eHealth Login button at the top of the page. Then enter your username and the password that you created when you registered.

What Can You Do With the Patient Portal?

Sign in to pay a bill, view statements, switch to paperless billing, and more. Or you can pay as a guest. Please use the online BILL Pay for statements dated after 2/1/2020 which will include the provider ID Number

Can you do a virtual visit with telehealth?

Request a new appointment: Face-to-face virtual office visits through Telehealth or phone call visits are available in addition to office visits. Your provider’s office will help you decide which type of visit is right for you.
