Medical records are confidential and are protected by state and federal laws. Although the medical record is the property of San Antonio Regional Hospital, the patient has the right to access information contained in their medical record.
You may obtain a birth certificate by submitting an application at the San Bernardino County Recorder Hall of Records, 222 W. Hospitality Lane, San Bernardino, CA 92415-0022. The Hall of Records is open Monday – Friday, 8:00 am to 4:30 pm; closed weekends and holidays.
If you would like a copy of your radiology studies, you can request a copy following the same process as above. Copies of radiology images are provided by the Health Information Management Department at the main hospital. Please complete the authorization form and send to the Health Information Management Department.
