What is CareSpace portal?

The CareSpace portal will allow give patients and their caregivers the ability to see their health information and communicate with their providers anytime, anywhere. SeeYourChart is replaced with CareSpace with better functionality and information.

How do you set up a CareSpace account?

Getting Started Follow these three steps to set up your account. Check your email for a registration link from CareSpace and our practice. Create a password for your CareSpace account. Log in using your email, password and date of birth.

What is the CareSpace app?

CareSpace provides tools and virtual coaching to help you make positive changes towards improving your health and wellness. The goal of the CareSpace app is to reduce your risk of type 2 diabetes and help you take steps to improving your overall health.

How do I register my Carespace portal?

Patients wishing to register for the patient portal, may ask any employee for assistance while in the office. You may call our office at 515-282-2921 and ask to speak with the clinical analyst in charge of the portal during office hours (8AM-5PM) also.
