Does Medicare Part B cover physicals?

Medicare Part B covers an annual visit, called a Medicare Annual Wellness Visit. These visits usually involve questions about your health, and family history, advice on prevention activity, and mental health screening. The Medicare Annual Wellness visit does not include a physical exam.

Can you refill prescriptions after hours?

There are certain medications with a high risk of side effects that must be authorized by your primary doctor. To ensure patient safety, our on-call physicians cannot provide refills for such medications after hours, over the weekend, or on holidays. Prescriptions can be filled through the Care New England Pharmacy.

Can you order lab work for x-rays?

As part of routine health care, it is very common for physicians to order lab tests such as blood work, or imaging studies such as x-rays. We certainly understand that waiting for these test results can be stressful, and it is natural to want to know the results as soon as possible.

What does a geriatric consultant do after a patient consultation?

Following the patient consultation, the physicians send a detailed summary of findings and recommendations to the referring physician. For more urgent matters, the geriatric consultant communicates directly with the referring physician by phone.

What is a geriatric consultation?

The goal of the consultation is to provide the patient, caregiver, and patient’s primary care provider with: 1 An in-depth assessment focused on a specific condition or general concerns (i.e. geriatric assessment). 2 A review of the patient’s current plan and medication regimen. 3 Advice about evaluation and management. 4 If appropriate, recommendations for additional tests or further evaluation.

What are some examples of planned offerings?

Examples of planned offerings include exercise classes (yoga, chair yoga, tai chi), fall prevention and recovery workshops, group visits for certain conditions or for education (diabetes management), patient and caregiver education, nutritional programs, and more.

How to get CNME to send notifications?

Step 1: Log in to MyChart. Step 2: Go to "Account Settings" and click "Communication Preferences". Step 3: Under "Health" deselect emails and/or text messages for test results. CNme does not currently push portal notifications to your phone or device.

What is HIE in healthcare?

Health Information Exchange (HIE) allows health care professionals to appropriately access and securely share patient medical information electronically. HIE systems have been in development nationwide since a federal law was passed in 2009 to promote the use of electronic movement and use of health information among health care providers.

What is the 21st century cures act?

The 21st Century Cures Act expanded the rights of patients to electronically access their medical history and treatment information. This prohibits the practice of restricting your access to your electronic medical information. I already use my Patient Portal (MyChart or CNme) to access my visits and test results.

What is clinical information?

Clinical information including medications, allergies, lab and test results, image reports, conditions, diagnoses and vaccination history. Certain demographic information used to identify the patient, such as name and birth date. How to opt-out of sharing health information with an HIE.

What information can you give to an emergency medical team?

In an emergency, you can quickly give emergency personnel vital information, such as a disease you're being treated for, previous surgeries or hospitalizations, medications you take, allergies, and how to contact your primary care doctor.

Can you opt out of a CNE?

Opting out of a CNE HIE will not adversely affect treatment by any Care New England treating provider, and patients cannot be discriminated against for choosing to opt out. If you change your mind about participating in a CNE HIE, you can opt back in at any time.
