New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Billing

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise. We reserve the right to charge $25.00 for “no-show” appointments.

Referrals

Some insurances may require a referral for your visit to our office. Please make sure to contact your insurance company and/or your primary care physician to ensure that you do get a referral if it is required.

Medical Records

Due to Federal HIPAA Privacy Regulations, a medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number.

Where is Capital Health located?

Capital Health Primary Care – Princeton, part of Capital Health Medical Group and a trusted provider of primary care in eastern Mercer County, moved to a new office at 300 Witherspoon Street, Princeton, NJ 08540. Capital Health’s primary care office in Princeton was previously located in Montgomery Commons, a few miles north of the downtown area.

What is Capital Health Post Acute Care?

To help bridge the gap from acute care to the varied levels of care outside the hospital, Capital Health has launched a new Post-Acute Care Program that is part of Capital Health Medical Group.

No appointment needed - just walk in! Patients are also seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call (850) 894-2401 during regular business hours. Please call in advance for office visits. Make follow-up appointments as you leave.
