What is Capital Health’s hospital patient portal?

To enroll in mycareDOT™, you must first be an established patient of Capital Healthcare Associates. You will need to provide our office with your name, email address, date of birth, and address. We will then send you an email inviting you to join the portal. Once you sign up, you can begin communicating with our office!

What is the patient portal and how do I access it?

To access your records for dates of service after February 6, please register for the new portal by visiting https://capitalhealth.iqhealth.com/self-enroll/. In order to self-enroll, you will need your Community Medical Record Number (CMRN) which can be found on your discharge instructions provided after your visit.

Who can sign up?

Patients and their legal guardians can sign up for Patient Portal in accordance to the Terms and Conditions of the website. Just tell your provider who you would like to enroll and you are on your way to connecting with your provider.

You can sign up and register for the Patient Portal by giving your provider your name and email address. In turn, your provider will give you an instruction sheet that includes a temporary username and password. The temporary credentials will allow you to log in to the system and create your own private user name and password.
