If you are an authorized caregiver that needs help accessing a family member’s account, you will need to contact that family member’s medical provider for support.
You will need to contact your medical provider’s office for any technical issues. But before you do, please check that you’re attempting to access your patient portal from a desktop computer using a Chrome, Microsoft Edge, Mozilla Firefox, or Safari browser. This may solve the problem. If not, your medical provider can supply additional assistance.
Test results can be accessed and reviewed through your patient portal. If you’re having issues locating your patient portal, the portal locator will help you find it.
Retrieve and manage your health information online anywhere, anytime with your computer, tablet, mobile device or smart phone with Family Medical Center’s new patient portal, FollowMyHealth™. If you’re a patient of Family Medical Center, you can use FollowMyHealth to review your medical information online and communicate with your Family Medical Center office. This is a FREE service provided by Family Medical Center. In order to create your account, you’ll need an invitation code (usually the last four digits of your social security number). Please contact our office to enroll and receive your invitation email.
The Follow My Health website is secure and configured in such a way that hackers cannot access the information.
How do I get a visit set up? A telehealth visit can be scheduled by calling the office or contacting your provider through the portal. You may even be simply sent a link through text or email asking you to schedule a telehealth visit in response to a call or question you sent. FMA providers are using the doxy.me HIPPA compliant platform and you need only a smart phone or computer with microphone and camera to participate. In addition, you need to be in either Massachusetts or Rhode Island during the time of the visit.
What is telehealth? Telehealth is a way to have medical care provided by your physician over video chat. Video chat is preferred because sometimes even if it doesn’t seem it, more can be gained by looking at you than just over the phone. Plus, we are all missing social interaction these days so it is a nice way to connect with your family doctor.
Is this considered equivalent to an in-office visit? Yes. Just as an in-office visit is scheduled you will be given an appointment time. The link you will need can be found in the section below, to be used ONLY at your designated appointment time or when requested by your provider.
Choosing a primary clinician on MyMedicare.gov does not change your Medicare benefits or which clinician you can see. It gives your provider access to more tools or services to help coordinate your care, especially when the clinician participates in an ACO or other Medicare alternative payment models.

No. HEALTHeChart is not an email system and we do not recommend sending any confidential information through standard Internet email. When you activate your personal HEALTHeChart account, you will be asked to provide your email address so that you can receive email notification to log on to your secure HEALTHeChart account.
Visit stvincentcharity.com and click the HEALTHeChart link on the homepage. Complete the online sign-up form by entering the requested information. (Please note that the information you provide is confidential and it will be processed through our secure website.) Your request will be reviewed and your information verified; then a special, one-time-use HealtheChart activation code will be emailed to your personal email account. Once you receive your one-time-use activation code, please use it immediately.
There is no charge. HEALTHeChart is offered complimentary to St. Vincent Charity Medical Center patients.
