The Patient Portal provides a secure way to get connected to your health information online. The Portal allows you view your lab work, receive notifications/health reminders from your provider, receive a summary of your health record, and much more!
We are currently welcoming new adult patients at Callen-Lorde Brooklyn and young adults ages 13-21 at our Health Outreach to Teens (HOTT) Program in Callen-Lorde Chelsea.
Patient portal apps typically allow patients to share their health records with providers, enabling them to gain a longitudinal view of their patient's health history. Like Epic's MyChart, some apps even allow patients to provide temporary access to their medical records via a code.

For uninsured/underinsured patients, Callen-Lorde provides a sliding fee based on your income. Visit co-pays can range from $0 to $100. By filling your prescriptions at a Callen-Lorde pharmacy, you help us provide care to the most vulnerable members of our communities, regardless of ability to pay.

There are two main types of patient portals: a standalone system and an integrated service. Integrated patient portal software functionality usually comes as a part of an EMR system, an EHR system or practice management software. But at their most basic, they're simply web-based tools.
To request your medical records, please fill out a HIPAA Form and email to records@callen-lorde.org. You can also pick up and/or submit a form at any front desk. Please note this process can take 5-7 business days.
