Current patients are welcome to call (803)364-4852 at any time to set up an appointment. Lovelace Family Medicine is proud to offer same day appointments with each of our providers in order to accommodate our patient’s busy schedules and to still allow them to see the provider of their choice.
In the event your provider feels it is necessary refer you to a specialist we ask that you give our office 7-10 business days to set up your appointment. If you have not heard from us or the specialist’s office after that time please call our office to check on your referral.
In order to keep your medical information confidential, medical records will only be released once our office has received a signed authorization form. You can stop by the office and request a form to fill out or you may print the form at the link below and mail, fax, or drop the form off at the front desk.
