The MyHealth Patient Portal is a website that offers personalized and secure online access to portions of your medical record. It enables you to securely use the internet to help manage and receive information about your health. MyHealth only displays patient information that occurred on or after October 30, 2016.

Use Self-enrollment Self-enrollment is available for Blanchard Valley Health System (BVHS) patients who are age 18 or older that don't currently have access to the MyHealth patient portal, or have never been emailed an invitation. Complete this form to determine if you are eligible for the self-enrollment process.
To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.
At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records.
