New users can register for AveraChart or log in if you already have an account.

How do I add someone to my Avera chart?

Select the Update User Preferences button. Select the down arrow, and select a patient from the list. Submit the change. This patient's record will automatically be displayed the next time you log in to AveraChart.

How do I add a child to my Avera chart?

AveraChart lets you view your child's or another adult's health information and you can give another adult access to your AveraChart account. Child age restrictions apply. Please download the proxy form, complete and return to your clinic or fax to 605-322-7750.
