Once they have created their own account, they can request access to your account. You then will receive notice of the request along with an auto-generated PIN code to share with the requesting individual. Once the requesting individual has entered the PIN code, the accounts will be linked and the requestor will be granted access to view your health information.

To obtain your own medical records: Print out the Consent to Release of Information form, and complete as many areas as you are able. Bring this completed form to the medical records department, and you can pick up your records.
Medical record pickup: 8 a.m. to 4:30 p.m. or by appointment.
To obtain records for a minor: Regardless of custodial arrangements, either parent may sign the authorization to obtain records of their child.
