What to do if you don't have a patient portal?

If you do not have a Patient Portal account, alert our registration staff members at your next clinic visit and they will help you sign up for the Patient Portal. How old do you have to be to get proxy access?

Proxy access can be established by family members for patients of all ages. Patients over the age of 15 are required to give consent to their family members before proxy access to their account will be granted.

Does MyChart have an activation code?

No, your activation code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. (The code will expire after you have used it or after 30 days). When you log into MyChart the first time, you will then be asked to create your own unique MyChart ID and password.
