Terms and Definitions

Throughout this Agreement you will see the terms “Website,” “Portal,” and “Patient.” The term “Website” refers to the Boise State University Health Services “Health Services” internet web address located at https://www.boisestate.edu/healthservices where our “Notice of Privacy Practices” can be found.

Acceptance and Participation

It is important that you read and understand all of the following terms and conditions carefully. You understand and agree that this Terms of Use Agreement is a legally binding agreement between you and Health Services.

Medical Emergencies

You understand that the Portal is not meant to be used in the case of an emergency. For all matters requiring urgent care which you believe may negatively impact your health or well-being, you understand that you must call 911 or proceed to the emergency department.

Eligibility for Use

All patients that are over the age of 18 that have received healthcare at Health Services are eligible to use the Patient Portal.

No Access for Minors

You understand that the portal is not available for use by persons under 18 years of age. If you are under 18 years old, please contact Health Services at (208) 426-1459 during our normal business hours to discuss alternative methods to send/receive information contained within your electronic health record.

Health Services Patient Portal Access and Use

You understand that the Portal is intended to be used only by individuals who wish to use the services and functionality that the Portal provides in connection with their health and well-being. You understand that all communication through the Portal will be in regard to your own health care needs and treatment.

General Information Is Not Medical Advice

The Content on the Health Services Website, or the Portal is NOT considered medical advice or treatment rendered and is not intended in any way to be a substitute for direct patient care.

During Your COVID Testing Appointment

Immediately alert staff if you are experiencing any signs or symptoms of COVID. Individuals are required to wear a facial covering and observe physical distancing when visiting the testing center. Be ready to present your Boise State campus ID or state identification card when requested.

Receiving your results

After testing you should receive an email when your test result is ready in 1-3 business days. This email will come from “noreply-clinicallab@stratusdx.net” with the sender name “Boise State”. The email will include the language “print or download the Result Report” and contain a link to sign-in to the Patient Portal.

Insurance billing

Boise State University COVID Testing Center will bill your insurance for your COVID test, but you will not be charged a co-pay or be responsible for any out-of-pocket costs related to testing. Any unpaid paid balances from your insurance company will be written off by Boise State. If you do not have insurance, we will not bill you for the test.

Parking

Limited complimentary parking is available in the parking lot located northeast of Campus School, or in the Brady Street Garage during your appointment. Additional pay-to-park options are available in the Brady Street Garage.

Community partner signup

The Boise State COVID Testing Center works with organizations and businesses in the community to facilitate surveillance testing or processing of samples collected from their members or employees.

Testing availability

Testing at the Boise State COVID Testing Center remains available and is encouraged for any campus community member who has concerns about symptoms or possible exposure.

Testing exemptions

In certain cases, on-campus residents may be granted temporary exemption from testing. To find out more about exemptions, visit the testing exemptions webpage.

About the test

The Boise State COVID Testing Center administers a saliva based RT-PCR diagnostic COVID-19 test that will show if you have an active coronavirus infection and need to isolate yourself from others.
