The relative mortality rate was high early after transplant as expected (standardized mortality ratio [SMR], 34.3 in the first 2-5 years) but persisted beyond 30 years (SMR, 5.4). Factors estimating mortality included age, high-risk disease, chronic GVHD, and use of PBSC grafts.
A 2016 study of over 6,000 adults with AML found that people who received an autologous bone marrow transplant had a 5-year survival rate of 65%. For those who received an allogenic bone marrow transplant, it was 62%.
Bone marrow donors do not ask for payment but the procedure itself is expensive, close to £10,000 per procedure. This includes donor's travel to the hospital, accommodation and other expenses, medical fees and insurance. The total cost of a bone marrow transplant is usually around £15,000.
If an allogeneic stem cell transplant is done during first remission, the 5-year disease-free survival rate is 30%–50%. If there has been no recurrence at 2 years after the stem cell transplant, the person has about an 80% chance of staying in complete remission for a long period of time.
During the procedure, the patient does not have much pain. A small incision is made, through which a wide bore needle is inserted into the bone marrow and stem cells are collected in syringes. The donor may experience pain after the anesthesia wears off. Painkillers may be needed for next few days.
Some 62% of BMT patients survived at least 365 days, and of those surviving 365 days, 89% survived at least another 365 days. Of the patients who survived 6 years post-BMT, 98.5% survived at least another year.
Still, it's not very common. Graft failure can lead to serious bleeding and/or infection. Graft failure is suspected in patients whose counts do not start going up within 3 to 4 weeks of a bone marrow or peripheral blood transplant, or within 7 weeks of a cord blood transplant.
For many patients, the donor search takes as little as 1-2 weeks from the time a donor testing request is made until the lab has the blood sample and HLA typing results. Sometimes it can take longer including some searches that may require multiple rounds of donor selections.
A transplant costs between £50,000 and £120,000, but the cost of caring for one patient refused a transplant in those few years was £130,000 for the year they survived, and for another patient who survived three years it was £160,000.
Mayo Clinic is one of the largest providers of bone marrow transplants in the United States. Mayo Clinic's bone marrow transplant teams in Arizona, Florida and Minnesota are leaders and innovators in a range of bone marrow transplant options: Cell therapy, with on-site stem cell processing laboratories.
The cells that are usually affected by GVHD are the skin, digestive tract, and liver. When GVHD starts, it is called acute GVHD. If it lasts for more than 100 days, or starts 100 days after your transplant, it is called chronic GVHD. Chronic GVHD can affect your eyes, mouth, lungs, vaginal lining, muscles, and joints.
When the new stem cells multiply, they make more blood cells. Then your blood counts will go back up. This is one way to know if a transplant was a success.
