Patient Portal Authorized Representatives

For Patients Who Are Minors: Due to federal and state confidentiality laws, patients who are between the ages of 14 and 17 are not required to disclose certain types of medical information to parents/legal guardians.

On January 4, 2021, Beaumont Family Practice began accepting appointments for Covid-19 vaccinations. As of today, January 8, 2021, the vaccine allocation received by Beaumont Family Practice has been scheduled out in full and all vaccines available are now accounted for.
