Through the Bluestone Bridge, you can communicate directly with your provider team. Access to the Bridge is through the purple button at the top of the site. Patient Portal: Access available to Patient or patient’s Designated Healthcare Agent. The Patient Portal is a separate platform allowing additional access to personal health information.

Accessing your medical record, managing your appointments, and communicating with your therapist

If you are a current client of a therapist working out of the Bluestone offices, you have the ability to access your chart through our electronic health records system. You may exchange encrypted emails with your therapist, update your chart information (such as change of address or insurance), and manage your schedule.

Bluestone Physician Services’ unique, robust model of care goes beyond primary care services. Our model is tailored to patients living with multiple chronic conditions and disabilities. What time does Bluestone Bridge return messages?

Send a message to your provider on the Bluestone Bridge. Providers return messages Monday-Friday, 8 a.m.-5 p.m. Messages received outside of normal business hours are also monitored by the on-call provider.
