How do I get my medical records from Benefis?

Release Your Medical Records The walk-in office at Medical Records located on Benefis East Campus is open for on-site records requests and pickup services from 8 a.m. to 5 p.m. Monday through Friday. While the office is open, you may also download and complete our "Authorization to Release Medical Information" form below.
