Beaver Medical Group launched a new portal called MyChart on October 30, 2019 at https://www.mybeaverchart.com . On the new portal, you are able to: See your medical record including allergies, medical history, problems, immunizations, medications, surgeries, etc.

You may also contact the Patient Service Advisor at (800) 831-0001 for information about physicians or to arrange a transfer from one Beaver Medical Group primary care physician to another.
If you are a new patient, you will need to complete a medical records release form to have your chart transferred from your previous doctor to Beaver Medical Group. Please complete the form soon after you join Beaver Medical Group so your physician has your complete medical history. You may download our Medical Records Release form , fill it out and fax it to your previous doctor. Learn more about medical records .
Allow 4-7 days for prescription refills. Use the Beaver Medical Group online patient portal for requests, or call the pharmacy where you filled the prescription; the pharmacy will send a request to your doctor for a new authorization if you do not have any remaining refills. If you have not seen a Beaver Medical Group doctor yet and need a refill, call to make an appointment and bring all your medications with you.
