If you are a new patient, you will need to complete a medical records release form to have your chart transferred from your previous doctor to Beaver Medical Group. Please complete the form soon after you join Beaver Medical Group so your physician has your complete medical history. Allow 4-7 days for prescription refills. Use the Beaver Medical Group online patient portal for requests, or call the pharmacy where you filled the prescription; the pharmacy will send a request to your doctor for a new authorization if you do not have any remaining refills. If you have not seen a Beaver Medical Group doctor yet and need a refill, call to make an appointment and bring all your medications with you.
