The Health Information Services (HIS) department at Beaufort Memorial Hospital fulfills requests for information for all inpatient, outpatient and clinical services. The HIS staff is happy to assist you in obtaining your medical records upon receipt of a valid authorization. You can mail, fax or personally deliver your authorization to release health information.

Beaufort Memorial Hospital. Beaufort Memorial, a not-for-profit hospital founded in 1944 on the banks of the Atlantic Intracoastal Waterway, is licensed for 197 beds (169 acute, 14 rehabilitation and 14 mental health).
Internal Medicine. Beaufort Memorial internists work mostly with adults age 18 and older. Just like other primary care providers, they diagnose and treat diseases, such as diabetes, congestive heart failure and high blood pressure, and they help you prevent these conditions if you are at high risk for developing them.
Help prevent future illness: When you visit your primary care provider for regular physicals and exams, you will learn more about topics specific to your health and how you can prevent chronic illnesses.

Beaufort Memorial also offers a number of ways to get care outside your provider’s office. Our Beaufort Memorial Express Care and Occupational Health Clinics are open after hours and on weekends. BMH Care Anywhere, our telehealth platform, lets you to schedule video visits with your primary care provider. You can also access healthcare providers who can assess your condition 24 hours a day, seven days a week.
To ensure coordinated quality care, medical information is often exchanged between health care professionals. This includes sending records to specialists, emergency departments, other hospitals, and more — with your request and authorization, of course. Or perhaps you want a copy of your medical record for personal reasons.
