I understand that the MyChart® service is available only to patients under the active care of a Baylor Clinic physician and will be deactivated once I am no longer a patient. I agree to contact Baylor Clinic once my care at Baylor Clinic ceases.

Baylor Medicine is committed to vaccinating our patients as quickly as we can. We are following the state guidelines for distributing the COVID-19 vaccine. You do not need to be a current Baylor Medicine patient to receive a vaccine. As of Tuesday, May 11, 2021, everyone aged 12 and older is now eligible to receive a COVID-19 vaccine.

How do I use MyChart?from bmc.org

To sign up for MyChart, click here. You’ll be asked to fill out information about yourself, including your address, date of birth, and email address. Please note that it will ask for part of your Social Security number. This is necessary to verify your identity before we can share medical records. Or, your doctor will give you an activation code to sign up.

How long does it take to set up MyChart?from bmc.org

Once your account is verified, you will get an email with the information you need to set up your account. It may take up to two to three days for you to get this information. You can also set up a MyChart account by calling 844-625-1390 or calling your provider’s office.

Is BMC releasing notes?from bmc.org

Because of a new federal rule, BMC will be releasing notes and results without a time delay to MyChart. As of November 16, you, and anyone you've given MyChart access to, will see test results without a time delay, a summary of what you told the doctor or nurse, findings from exams, your doctor's assessment or diagnosis of any medical conditions, and your treatment plan and next steps. The Adult to Adult proxy requires the patient to provide a picture ID as a support document with additional evidence that the patient consents to share his/her private health information with the proxy.

Can MyChart be tracked?

I understand that my activities within MyChart® may be tracked by computer audit and that entries I make may become part of the electronic record.

Does MyChart reflect the complete medical record?

I understand that MyChart® contains selected, limited medical information from a patient’s medical record and that MyChart® does not reflect the complete contents of the medical record. I also understand that I may request a paper copy or a summary of my medical record from the Baylor Clinic.

Can I use MyChart for emergency?

I understand that MyChart® is never to be used as a means of communication to Baylor Clinic health care providers for urgent or emergency matters. For all emergency matters that I believe might immediately affect my health or well-being, I will dial 911 without delay and/or go to the emergency department of a local hospital.

Is MyChart a secure website?

I understand that MyChart® is intended as a secure online source of confidential medical information. I must have internet access and my computer’s internet browser must be either Mozilla Firefox, Safari, or IE 6 or newer.

Is MyChart available at Baylor?
