How To Request Your Medical Records

It’s important that you have easy access to all your medical records. Staying informed about your diagnosis and treatment is a critical part of your care experience at Barrow Brain and Spine.

Step One: Completing Your Form Request

You will first need to complete the Barrow Brain and Spine “ Authorization for Release of Protected Health Information ” form.

Step Three: Awaiting Your Records

Due to the high volume of requests, expect to receive your records within 30 days.
