Click the Patient Portal Log-in button above. Choose the "Sign up today” to create a new account. Enter your name, date of birth and contact information. Create a password. This password must be 8-20 characters and include one upper and one lower case letter, and one number or symbol.

Yes. Using the portal locator doesn’t affect your ability to use other URLs to log in to your patient portal.

It is at your provider's discretion to make test results available. Your provider must authorize the release of your test results in order for them to post to your Patient Portal account. Only test results which are considered appropriate for release will be accessible through the Patient Portal. top

Is Concord Hospital a separate portal?from concordhospital.org

The patient portal for Concord Hospital - Laconia and Concord Hospital - Franklin is separate from My Patient Connect. In the future, the two portals will be combined but for now, patients need to log in to separate portals based on where they receive their care.
