You submit a questionnaire, medical records and a course of treatment and your provider will message you with next steps. An e-visit costs $35. To request access to Baptist Gateway, simply complete the BAPTIST GATEWAY PATIENT PORTAL ACCESS REQUEST FORM. This request form must be sent to BaptistGatewaySupport@BaptistFirst.org from a valid email address (cloud messages are not accepted) or returned in-person to the facility's Health Information Department from which you are requesting your records.
