What are the benefits of patient portal?

The My Baptist Connect website is a safe, private way for Baptist Health patients to stay connected to their personal health information. When you enroll in My Baptist Connect, you’ll be able to view: Clinical summaries of your hospital visit Your recent medical history including prescriptions Your hospital discharge instructions

What is a portal patient?

MyChart is a free patient portal that combines your Baptist Health medical records into one location. The easy-to-use online tool helps you manage your health by connecting you with providers and giving you access to lab results, appointment information, video visits with providers, current medications, and more from your computer, tablet, or mobile device.

What is patient portal?

Call 904.271.6000. Ask for the patient by name. Patients may receive calls 7 am - 10 pm. Only emergency calls will be accepted after hours. For the consideration of patients, please turn off cell phones while in patient care areas. Ample parking is available in our flat lot parking and our four-story parking garage.

What is My Baptist Connect?

The My Baptist Connect website is a safe, private way for Baptist Health patients to stay connected to their personal health information.

Enrollment is easy and free

To enroll, all you need is a secure Personal Information Number, or PIN. We’ll send you a secure PIN by mail after your visit — or we can email it to you as a part of your registration process, if you prefer.

How to Get an Activation Code

If you would like to sign up and don’t have an After Visit summary from your physician or a Baptist Health facility, visit MyChart to get an activation code. Once you have your activation code, you can setup your username and password on the MyChart website .

Login to MyChart

To login to MyChart, visit mychart.baptisthealth.com. Once on the page, enter your username and password that you setup when your created your account and click Sign In.

How to Use MyChart

After logging into your MyChart account, hover over the Messaging tab and select “Ask a Question”

US Mail Boxes

There is a mail box located between the hospital and the Medical Office Building 1.

Spiritual Care and Chapel

Two full time chaplains, a chaplain resident and chaplain Interns, along with CPE trained volunteer chaplains are available to provide spiritual and emotional support to you and your family. Our chaplains minister from a multi-faith perspective and can assist in contacting a faith representative or resource of your preference.

Medical Ethics Committee

This interdisciplinary committee offers guidance to families, providers and treatment teams who are trying to reach an agreement about the best way to care for a patient, helping to provide clarity in cases where difficult ethical issues arise. Consult the Medical Ethics committee when:

Discharge Planning

Our social services department can help you plan your discharge, home health and medical equipment needs, community services, counseling, transportation and more. Ask your nurse or call the hospital operator at 904.271.6000 and ask to speak to a social services representative.
