How to make an appointment at a clinic?

Making an appointment at a clinic is now easier than ever. Use the MyChart patient portal to schedule an appointment, request an appointment or cancel an upcoming visit with your primary care physician. Prior to your scheduled visit with your provider, you can use eCheck- In to update insurance information, medications, allergies, fill out questionnaires and sign documents.

How much does an e-visit cost?

You submit a questionnaire, medical records and a course of treatment and your provider will message you with next steps. An e-visit costs $35. It is a flat rate fee payable by credit card or Health Savings Account (HSA). If the provider determines that you need to schedule an in-person visit, your card will not be charged. The $35 fee does not include the cost of other services such as laboratory testing, prescription medications, or x-rays, if any are needed. Your provider will not refill medications, address chronic conditions, or prescribe controlled medications through an e-visit.

Can you send a letter to your primary care provider?

You can send a message to your primary care provider if any follow up questions arise regarding a current health concern. You will also be able to receive letters when needed, including school excuses, work excuses or referral letters.

How to access Baptist Gateway?

To request access to Baptist Gateway, simply complete the BAPTIST GATEWAY PATIENT PORTAL ACCESS REQUEST FORM. This request form must be sent to BaptistGatewaySupport@BaptistFirst.org from a valid email address (cloud messages are not accepted) or returned in-person to the facility's Health Information Department from which you are requesting your records. Please allow 7 to 10 days to process your request.

You can also download a copy of your My Baptist Health records at any time in either an Adobe® PDF or XML file format. Simply click on the “Download My Data” icon on the My Baptist Health home page, which will take you to the “More Options” page. Select the “Upload/Download Health Data” link. Your health record will automatically be displayed on the “Upload/Download CCD Format Health Data” page. Use the dropdown menu at the bottom of that page to select either PDF or XML. Then click the blue “Download My Data” icon. If required by your browser, click “Yes” to view the exported file.

You can create a MyChart account by visiting MyChart.BaptistHealth.com or calling 502.253.4820 or toll-free at 1.844.7820.

Does my Baptist Health roll over to my chart?

Your My Baptist Health healthcare records will not roll over into MyChart. However, while no new information will be added, you can continue to access My Baptist Health via the ID and password you use currently and use its content as an archive of your healthcare records rather than as an active chart.
