Use the MyChart patient portal to schedule an appointment, request an appointment or cancel an upcoming visit with your primary care physician. If you would like to sign up and don’t have an After Visit summary from your physician or a Baptist Health facility, visit MyChart to get an activation code. Once you have your activation code, you can setup your username and password on the MyChart website .
To login to MyChart, visit mychart.baptisthealth.com. Once on the page, enter your username and password that you setup when your created your account and click Sign In.
After logging into your MyChart account, hover over the Messaging tab and select “Ask a Question”
Complete the check-in process from home to save time when you arrive at the clinic. Pay visit copays, verify your medical and insurance information, respond to questionnaires, and electronically sign documents.
Patients must use MyChart mobile version 9.3 or higher to log in to the MyChart app.
