If you would like to sign up and don’t have an After Visit summary from your physician or a Baptist Health facility or an activation code, you can still sign up for MyChart. If you've been a Baptist Health patient, your MyChart account will connect to your medical record.
You also may import information from MyChart into other applications that, with your permission, can use that data to help you manage your care, medications, nutrition and more.
Improper activities include, but are not limited to, the use of inappropriate or abusive language, reserving appointment times that are not needed or are frequently cancelled and other activities that are deemed to cause a disruption to operations in the sole discretion of Baptist Health.
The access code you receive will give you the opportunity to create a personal login name (called “ MyChart ID ”) and a password . Once this access code has been used it will no longer be valid. It is extremely important that you keep your login name and password completely confidential.

On 10.17.2020 I was taken to Baptist East E R, afterwards I was admitted to Room 532 5 South. All of the Staff Doctors, Nurses, Nursing Assistant, Dietary and House Keeping. Was here for kidney stones, tgen they wanted to do observation, Paula from billing immediately told me to leave and go to the V.A. hospital. I wanted to stay because the V.A. covers me 100 percent, but billing wanted me out. This bothers me as a veteran of 25 years I wasn't good enough to get care here.
