You must be 18 years or older to use self-enrollment. Self-enrollment is for patient use only. You will need your medical record number to enroll. When can I expect to receive my invitation to Aultman OneChart?

You will receive a reminder email if you have not set up your account after seven days. The invitation will expire after 90 days. Please email patientportal@aultman.com if your invitation has expired and a new invitation will be sent. This email address is monitored 8 a.m. through 4:30 p.m. Monday through Friday. Please allow for a 24-hour turnaround time during business hours.

Can I access Aultman OneChart on my mobile device?

Yes, you can access the portal via your smartphone or tablet by going to www.aultman.org/AultmanOneChart.
