How long is a patient's authorization kept?

Your authorization is kept in our files for one year, from the date originally signed, unless revoked in writing by the patient.

Who does ASCS bill to?

ASCS will submit your bill to either the employer responsible or the workers compensation insurance company along with your medical records, as required by Minnesota State Law. You are required to provide the information (First Report of Injury) to us for billing.

How Do I Make An Appointment?

Call us at (763) 571-4000 to make an appointment for all locations. If there is a location that you would prefer or desire please specify the location when scheduling your appointment. We will ask that you describe the reason for your visit so we can schedule you with the appropriate dermatologist and for the adequate amount of time.

How Do I Refill My Prescription?

Call your pharmacist to refill a prescription. The pharmacist will then call our office for approval.
